Senior Services Care Coordinator. -This role allows you to use your
knowledge to educate and provide quality care to our patients. -
The Senior Services Care Coordinator supports our senior population
with health care plans and programs (Medicare Advantage, Methodist
ACO and Traditional Medicare), performs screening tests, and
conducts other tasks to ensure patients meet plan requirements.
Essential Skills:
- Handle incoming inquiries from team members regarding the
Senior Care population
- Conduct Focus of Care Testing on patients, order lab
requisitions and record results in patient charts
- Process lab requisitions
- Inventory medical supplies weekly and submit order to Care
Coordinator Lead
- Compile reports for senior population data mining using Excel
and insurance websites or correspondence by mail
- Call senior patients to schedule for testing, appointments and
education
- Desktop support- all items are processed each day - - -
- Provide coverage for other Care Coordinators when
necessary
- Coordinate training schedules for providers and team members
for outside vendors
- Send out communication emails to Village Providers, team
members and outside vendors
- Copy educational material for patients, providers, and team
members
- Conduct Attestations
- Attend meetings with team and outside vendors
- Travel to the different Village Health Care sites occasionally
required
- Position requires heavy patient contact in person and by
phone
- Other administrative duties as needed Medical Assistant
Qualifications:
2-3 years related experience
Certified Medical Assistant - must be active and current Excel
experience a plus E-Clinical Works a plus Professional, team player
with a positive attitude Experience in properly handling
confidential and sensitive information; knowledge of HIPAA rules
and regulations Experience with basic desktop software, medical
practice management, and electronic medical record systems We offer
