Mustang Creek Estates Assisted Living & Memory Care is looking for an amazing Community Wellness Directors to assist in delivering quality care to our senior residents. Position Summary The Community Wellness Director is responsible for the oversight of the resident care of multiple campuses. They will assess and monitor systems to ensure a quality delivery of services. This role is a support to local campuses and campus leadership. This will be 100% local travel in the DFW area. • Regular attendance of stand-up meetings
• Regular travel (drive) to Mustang Creek Estates campuses. Muti-campus responsibilities.
• Daily review of Electronic Medication Record (EMAR) and ensure that dashboards remain clear
• Create and distribute out of med reports with a review of the document and sent to campus Administrative Assistants
• Daily review of electronic record of care (POC) for campuses. Distribute unconfirmed tasks and reconcile for the Executive Director and House Supervisor within 24 hours.
• Weekly new resident check-in. This includes seeing the resident, ensure that POC is up and running and all medications are accurate and stocked
• Move-in Homework Packet is distributed and completed for all new move-ins
• New Resident audit within 7 days of the physical move-in
• Provide support for the Executive Director in regards to at-risk care services for residents
• Create and distribute as-risk reports for care services on the campus on a weekly basis
• State compliance audit support, as requested
• Conduct TB Tests for new Team Members and/or Residents
• Monthly resident change of condition assessments and update POC based on the residents needs
• Annual Resident Assessments, POC and updated Service Plans
• Develops and maintains an excellent relationship with internal and external clients to ensure achievement of the highest quality of care for our seniors
• Follows all applicable Mustang Creek Estates community, state and federal rules and regulations
• Other duties and responsibilities as assigned Qualifications
  • Must possess and maintain a valid Texas state nursing license (LVN or RN)
  • Minimum of 5 years relevant experience, preferably have an Assisted Living/Memory Care background
  • Ability to read, write and speak English fluently
  • Strong computer skills (Preferred Microsoft Suite, Outlook and Eldermark)
  • Must have a reliable vehicle for local transportation
  • Must have a valid Texas Drivers' License and ability to maintain state insurance coverage
  • Excellent customer service skills with a strong focus on hospitality
  • Ability to act in a resident's best interest
  • Ability to act and supervise in a professional and courteous manner
  • Must have excellent interpersonal and communication skills
  • Ability to follow MCE policies and procedures
